At Summerseat Physiotherapy Clinic Ltd, we are committed to protecting and respecting your privacy.
On appointment by you, there are certain pieces of personal information which we are required to take. This information is necessary for us to provide holistic and complete care.
This statement is written to explain to you what types of information we record about you, why we record this information, how we keep it secure, how long we keep the information for, and your rights.
The information that we take from you is used to ensure that we provide you with the best and most appropriate treatment. We will not ask for more information than is needed.
This information will include:
- Your name, address and contact details
- Your Date of Birth
- Any medical/medication history
- Familial support, carers or other professionals involved
Your information which will be a mixture of both paper and electronic records will be stored safely and securely either in locked cabinets or electronically under password protection. Any electronic information will be stored under strict data protection guidelines.
The information that we hold will be accurate and up to date and used for the sole purpose of treatment by us. At times it may be necessary for us to share your information with other professionals, family or carers according to the information provided by you. We respectfully ask that any change of details be communicated to your therapist to ensure your records are accurate.
We will keep your information for a minimum of 8 years from the date of discharge, as dictated by our professional guidelines. After this time the information will be destroyed as no longer needed.
As a person on whom we hold information, you have the right to ask us what information we hold, you can ask to see the information we hold, and you can ask us to cease to hold information about you, although this may affect the ongoing service that we can provide to you.
As a part of your assessment by us, you will now be asked to sign the relevant section on the assessment form to say that you have read and accept the terms of our privacy policy.
