We are Summerseat Physiotherapy Clinic Ltd, a team of Chartered Physiotherapists offering physiotherapy treatment services either at our clinic based in Summerseat, in our client’s home address or other community venue.

We obtain information about you when you contact us directly either by telephone, in person or via our website, for example, when you make an enquiry or subsequently, when you instruct us to provide physiotherapy services to you, to a member of your family or to someone on whose behalf you act.

To manage an initial enquiry we would need to take details about you such as your name, a preferred treatment address (for providing an estimate of charges), your contact number and/or email address and a brief medical history.

On registration with our Practice we would expect to hold information relating to you as follows:

  • Details about you such as your address, your date of birth, your carer, your Case Manager, legal representative and your emergency contact
  • Any contact that our Practice has had with you such as appointments, conversations etc.
  • Notes and reports about your treatment from us
  • Details about your care and support
  • Relevant information from other health professionals, relatives or those that care for you

 

How do we maintain confidentiality of our records?

We are committed to protecting your privacy and will only use information collected lawfully, in accordance with the Data Protection Act 1998 and GDPR Guidelines 2018.

Every member of staff working at Summerseat Physiotherapy Clinic Ltd has a legal obligation to keep information about you confidential.

We will only ever use or pass on information about you if others involved in your care have a genuine need for it. We will not disclose your information to any third party without your permission unless there are exceptional circumstances (i.e. life or death situations), where the law requires information to be passed on.

Who might you expect us to share your information with?

  • Other healthcare professionals
  • Our colleagues who may also hold a clinical duty of care to you

Access to personal information

You have a right under the Data Protection Act 1998 to request access to view or obtain copies of what information the Practice holds about you and to have it amended should it be inaccurate. Change of details

It is important that you tell the person treating you if any of your details such as your name or address have changed or if any of your details such as your date of birth is incorrect for this to be amended. You have a responsibility to inform us of any changes so that our records are accurate and up to date for you.
